Pilot-to-Production Governance Sprint

Your AI pilots are not failing because the technology is wrong. They are failing because governance was never built in.

Deloitte's 2026 State of AI in the Enterprise report surveyed 3,235 senior leaders and found that only 25% of organisations have moved 40% or more of their AI experiments into production. The barrier is rarely technical. It is governance, integration, and oversight: the work that was deferred until after the proof of concept, and never happened.

Why pilots stall

A proof of concept runs in a clean environment with prepared data and a small team who understand the system. It works. The results are positive. The decision is made to move to production. Then nothing happens for months.

The usual explanation is organisational friction or lack of executive buy-in. But that is rarely the root cause. The actual barriers are consistently the same across organisations: production deployment requires infrastructure that was not built, security reviews that nobody initiated, compliance sign-offs from teams who were never included, monitoring systems that do not exist, and accountability structures that were never defined. The pilot worked precisely because it avoided all of these. Production cannot.

Deloitte's 2026 research names this the proof-of-concept trap: organisations experiment with AI, see positive results in controlled conditions, and then cannot consistently predict which use cases will yield return on investment, because no one built the mechanism for moving from pilot to scale. One healthcare AI leader quoted in the report described it precisely: "Without a clear roadmap, executing a hundred pilots just leads to poor results and failed value creation."

The solution is not more pilots. It is building the governance and integration architecture that makes existing pilots producible.

What actually blocks production deployment

Based on consistent patterns across enterprise AI deployments, there are five categories of barriers that prevent pilots from reaching production. Each requires specific work to resolve.

Governance accountability gaps. Production AI requires a named owner for each system, a defined escalation path when the system behaves unexpectedly, and a documented review process. Most pilots have none of these. When a regulator, an auditor, or a board member asks who is responsible for an AI system's outputs, the answer needs to be specific and verifiable.

Regulatory compliance blockers. A pilot can run without triggering most regulatory requirements. A production system processing customer data, making decisions that affect customers, or operating within a regulated industry typically cannot. The compliance team's concerns are not bureaucratic obstruction. They are legitimate legal exposure that was not addressed during the pilot phase.

Integration complexity. Pilots frequently use cleansed test data and simplified interfaces. Production systems must integrate with live data sources, existing systems, access controls, and audit trail requirements. Models that achieved high accuracy in testing may prove inadequate when handling the edge cases that live data generates at scale.

Monitoring and oversight gaps. Production AI requires ongoing monitoring: performance metrics, anomaly detection, drift detection, and human review processes. Without these, the organisation cannot demonstrate that the system is working as intended, and cannot identify problems before they become incidents.

Security and data handling obligations. Production systems must satisfy information security requirements, data classification policies, and PDPA obligations. These are rarely scoped during the pilot phase and can require significant infrastructure work to address.

What the sprint produces

The engagement begins with a structured inventory of all AI pilots currently in the organisation, including those that have been formally paused, those that are nominally "in progress" with no clear path forward, and those that delivered positive results but never moved to production.

For each pilot in scope, the sprint diagnoses the specific barriers blocking production deployment and specifies what needs to be built to clear them. This is not a generic governance framework. It is a barrier-specific remediation plan for each initiative, with the governance architecture designed to close exactly the gaps that are holding that pilot back.

The output includes a board-ready production case for the two highest-priority initiatives: the governance posture, the compliance position, and the risk controls that make executive sign-off straightforward rather than contested.

The sequencing question

Not every stalled pilot should move to production. Part of the value this sprint delivers is a clear-eyed assessment of which initiatives are worth the governance investment and which should be retired. An organisation with twelve stalled pilots does not need to govern all twelve. It needs to identify the three with the highest potential return, build the governance for those, and make a deliberate decision about the rest. That prioritisation is part of the sprint output.
